I give the Fertility Institute of North Alabama (“FINA”) permission to use a photo, video, and/or written comments of me and/or my child(ren) — whether shared with FINA directly, posted in an online review, or taken by FINA — on FINA’s website, social media, and printed materials. I understand this is entirely voluntary, that declining will not affect my care in any way, that I will not be compensated for this use, that FINA will not pair any image with details of my medical history or treatment, and that if a minor child is pictured I am their parent or legal guardian and have the authority to consent on their behalf. I may revoke this consent at any time by submitting a written request to FINA at 808 Turner St SW, Huntsville, AL 35801 or appointments@fertilityalabama.com; revocation will not affect any use that already occurred, and FINA may not be able to remove copies already shared by others outside FINA.
